The ability to accurately identify human gait intent is a challenge relevant to the success of many applications in robotics, including, but not limited to, assistive devices. Most existing intent identification approaches, however, are either sensor-specific or use a pattern-recognition approach that requires large amounts of training data. This paper introduces a real-time walking speed intent identification algorithm based on the Mahalanobis distance that requires minimal training data. This data efficiency is enabled by making the simplifying assumption that each time step of walking data is independent of all other time steps. The accuracy of the algorithm was analyzed through human-subject experiments that were conducted using controlled walking speed changes on a treadmill. Experimental results confirm that the model used for intent identification converges quickly (within 5 min of training data). On average, the algorithm successfully detected the change in desired walking speed within one gait cycle and had a maximum of 87% accuracy at responding with the correct intent category of speed up, slow down, or no change. The findings also show that the accuracy of the algorithm improves with the magnitude of the speed change, while speed increases were more easily detected than speed decreases.
Patterns of enteric infections in a population-wide cohort study of sequelae, British Columbia, Canada

We assessed patterns of enteric infections caused by 14 pathogens, in a longitudinal cohort study of sequelae in British Columbia (BC) Canada, 2005–2014. Our population cohort of 5.8 million individuals was followed for an average of 7.5 years/person; during this time, 40 523 individuals experienced 42 308 incident laboratory-confirmed, provincially reported enteric infections (96.4 incident infections per 100 000 person-years). Most individuals (38 882/40 523; 96%) had only one, but 4% had multiple concurrent infections or more than one infection across the study. Among individuals with more than one infection, the pathogens and combinations occurring most frequently per individual matched the pathogens occurring most frequently in the BC population. An additional 298 557 new fee-for-service physician visits and hospitalisations for enteric infections, that did not coincide with a reported enteric infection, also occurred, and some may be potentially unreported enteric infections. Our findings demonstrate that sequelae risk analyses should explore the possible impacts of multiple infections, and that estimating risk for individuals who may have had a potentially unreported enteric infection is warranted.

Diets varying in SFA and MUFA content can impact glycaemic control; however, whether underlying differences in genetic make-up can influence blood glucose responses to these dietary fatty acids is unknown. We examined the impact of dietary oils varying in SFA/MUFA content on changes in blood glucose levels (primary outcome) and whether these changes were modified by variants in the stearoyl-CoA desaturase (SCD) gene (secondary outcome). Obese men and women participating in the randomised, crossover, isoenergetic, controlled-feeding Canola Oil Multicenter Intervention Trial II consumed three dietary oils for 6 weeks, with washout periods of ˜6 weeks between each treatment. Diets studied included a high SFA/low MUFA Control oil (36·6 % SFA/28·2 % MUFA), a conventional canola oil (6·2 % SFA/63·1 % MUFA) and a high-oleic acid canola oil (5·8 % SFA/74·7 % MUFA). No differences in fasting blood glucose were observed following the consumption of the dietary oils. However, when stratified by SCD genotypes, significant SNP-by-treatment interactions on blood glucose response were found with additive models for rs1502593 (P = 0·01), rs3071 (P = 0·02) and rs522951 (P = 0·03). The interaction for rs3071 remained significant (P = 0·005) when analysed with a recessive model, where individuals carrying the CC genotype showed an increase (0·14 (sem 0·09) mmol/l) in blood glucose levels with the Control oil diet, but reductions in blood glucose with both MUFA oil diets. Individuals carrying the AA and AC genotypes experienced reductions in blood glucose in response to all three oils. These findings identify a potential new target for personalised nutrition approaches aimed at improving glycaemic control.

A national need is to prepare for and respond to accidental or intentional disasters categorized as chemical, biological, radiological, nuclear, or explosive (CBRNE). These incidents require specific subject-matter expertise, yet have commonalities. We identify 7 core elements comprising CBRNE science that require integration for effective preparedness planning and public health and medical response and recovery. These core elements are (1) basic and clinical sciences, (2) modeling and systems management, (3) planning, (4) response and incident management, (5) recovery and resilience, (6) lessons learned, and (7) continuous improvement. A key feature is the ability of relevant subject matter experts to integrate information into response operations. We propose the CBRNE medical operations science support expert as a professional who (1) understands that CBRNE incidents require an integrated systems approach, (2) understands the key functions and contributions of CBRNE science practitioners, (3) helps direct strategic and tactical CBRNE planning and responses through first-hand experience, and (4) provides advice to senior decision-makers managing response activities. Recognition of both CBRNE science as a distinct competency and the establishment of the CBRNE medical operations science support expert informs the public of the enormous progress made, broadcasts opportunities for new talent, and enhances the sophistication and analytic expertise of senior managers planning for and responding to CBRNE incidents.

After five positive randomized controlled trials showed benefit of mechanical thrombectomy in the management of acute ischemic stroke with emergent large-vessel occlusion, a multi-society meeting was organized during the 17th Congress of the World Federation of Interventional and Therapeutic Neuroradiology in October 2017 in Budapest, Hungary. This multi-society meeting was dedicated to establish standards of practice in acute ischemic stroke intervention aiming for a consensus on the minimum requirements for centers providing such treatment. In an ideal situation, all patients would be treated at a center offering a full spectrum of neuroendovascular care (a level 1 center). However, for geographical reasons, some patients are unable to reach such a center in a reasonable period of time. With this in mind, the group paid special attention to define recommendations on the prerequisites of organizing stroke centers providing medical thrombectomy for acute ischemic stroke, but not for other neurovascular diseases (level 2 centers). Finally, some centers will have a stroke unit and offer intravenous thrombolysis, but not any endovascular stroke therapy (level 3 centers). Together, these level 1, 2, and 3 centers form a complete stroke system of care. The multi-society group provides recommendations and a framework for the development of medical thrombectomy services worldwide.

Background
Depression is considered to have the highest disability burden of all conditions. Although treatment-resistant depression (TRD) is a key contributor to that burden, there is little understanding of the best treatment approaches for it and specifically the effectiveness of available augmentation approaches.
AimsWe conducted a systematic review and meta-analysis to search and quantify the evidence of psychological and pharmacological augmentation interventions for TRD.
MethodParticipants with TRD (defined as insufficient response to at least two antidepressants) were randomised to at least one augmentation treatment in the trial. Pre-post analysis assessed treatment effectiveness, providing an effect size (ES) independent of comparator interventions.
ResultsOf 28 trials, 3 investigated psychological treatments and 25 examined pharmacological interventions. Pre-post analyses demonstrated N-methyl-d-aspartate-targeting drugs to have the highest ES (ES = 1.48, 95% CI 1.25–1.71). Other than aripiprazole (four studies, ES = 1.33, 95% CI 1.23–1.44) and lithium (three studies, ES = 1.00, 95% CI 0.81–1.20), treatments were each investigated in less than three studies. Overall, pharmacological (ES = 1.19, 95% CI 1.08–1.30) and psychological (ES = 1.43, 95% CI 0.50–2.36) therapies yielded higher ESs than pill placebo (ES = 0.78, 95% CI 0.66–0.91) and psychological control (ES = 0.94, 95% CI 0.36–1.52).
ConclusionsDespite being used widely in clinical practice, the evidence for augmentation treatments in TRD is sparse. Although pre-post meta-analyses are limited by the absence of direct comparison, this work finds promising evidence across treatment modalities.

Recent modelling estimates up to two-thirds of new HIV infections among men who have sex with men occur within partnerships, indicating the importance of dyadic HIV prevention efforts. Although new interventions are available to promote dyadic health-enhancing behaviours, minimal research has examined what factors influence partners’ mutual engagement in these behaviours, a critical component of intervention success. Actor-partner interdependence modelling was used to examine associations between relationship characteristics and several dyadic outcomes theorised as antecedents to health-enhancing behaviours: planning and decision making, communication, and joint effort. Among 270 male-male partnerships, relationship satisfaction was significantly associated with all three outcomes for actors (p = .02, .02, .06 respectively). Latino men reported poorer planning and decision making (actor p = .032) and communication (partner p = .044). Alcohol use was significantly and negatively associated with all outcomes except actors’ planning and decision making (actors: p = .11, .038, .004 respectively; partners: p = .03, .056, .02 respectively). Having a sexual agreement was significantly associated with actors’ planning and decision making (p = .007) and communication (p = .008). Focusing on interactions between partners produces a more comprehensive understanding of male couples’ ability to engage in health-enhancing behaviours. This knowledge further identifies new and important foci for the tailoring of dyadic HIV prevention and care interventions.

Trilobites from thrombolitic reefs in the Ore Hill Member of the Upper Cambrian Gatesburg Formation are assigned to (in ascending order) the Pseudosaratogia magna, Cliffia lataegenae, and Irvingella major Subzones of the Elvinia Zone. These reef faunas contrast markedly with those from coeval off-reef grainstones (tempestites), representing separate biofacies rather than biostratinomic variants. Associated lithologies suggest that the fauna of the P. magna Subzone at the base of the member comprises a biofacies associated with shallower microbial reef environments than those occupied by faunas in the overlying C. lataegenae Subzone. Reefs in the C. lataegenae Subzone yield a fauna dominated by Buttsia drabensis, extending the documented range of catillicephalid abundance in microbial reef biofacies downward into the highest strata of the Steptoean Stage. This reef fauna also includes several new taxa, including Cheilocephalus quadratus new species (described herein), an atypical species of Cheilocephalus with catillicephalid-like cranidial features. The fauna in off-reef tempestites low in the C. lataegenae Subzone contains the genera that characterize the Xenocheilus–Kindbladia Biofacies, expanding the known environmental and geographic range of that biofacies. Sulcocephalus, typically a minor component of faunas in the I. major Subzone, dominates the fauna in reefs within this subzone such that a distinct Sulcocephalus Biofacies can be recognized immediately above the extinction horizon that defines the base of the Sunwaptan Stage.

The Laurentian Great Lakes represent the world's largest freshwater ecosystem and contain irreplaceable biodiversity. Lakewide Action and Management Plans (LAMPs) hold the highest potential for ecosystem management in the Great Lakes but have not specifically addressed biodiversity status or strategies for conservation. For four Great Lakes, recently completed biodiversity conservation strategies (blueprints) have assessed the status and threats to biodiversity and recommended strategies for conservation and restoration; a blueprint is under way also for Lake Superior. Here, we compare the completed blueprints and explore challenges to conservation planning for large ecosystems. We also assess whether earlier blueprints are being adopted and offer suggestions for more effective implementation. All of the blueprints focus on biodiversity in the lakes and coastal areas, and some include tributaries and migratory species. Biodiversity status was rated as fair (out of desirable range but restorable) in each lake, with some exceptions and considerable spatial variability. Aquatic invasive species ranked as a top threat to biodiversity in all four blueprints. Other highly ranked threats included incompatible development, climate change, terrestrial invasive species, dams and barriers, and non-point-source pollutants. The recommended strategies are characterized by six themes: coastal conservation, invasive species, connectivity and hydrology, fish restoration, nearshore water quality, and climate change. Each blueprint highlights high-priority strategies, but successful protection and restoration of Great Lakes biodiversity require revisiting these priorities in an adaptive approach.

Changes in the composition and structure of mid-altitude, semi-deciduous tropical forest in Mpanga Research Forest Reserve, Uganda are described for a 25-year period between 1968 and 1993. Three surveys of a 0.64 ha (80 m × 80 m) permanent plot were carried out in 1968, 1982 and 1993, during which a total of 397 trees with a dbh≥9.5 cm were identified. Forty-nine species were identified in total, representing 19 families. Eight individuals remain unidentified. Dbh measurements were recorded for 359 non-buttressed trees, whilst the equivalent measurement for 38 buttressed trees was the diameter of the trunk immediately above the buttress. Basal area, diversity and density of trees increased within the plot during the survey period by, respectively, 8% (from 39.2 to 42.2 m2 ha−1). 7% (from 44 to 47 species) and 11% (459 to 508 trees ha−1). The main compositional changes were increases in understorey trees and a decline in serai taxa. Growth rates (productivity) and turnover were lower during the period 1982 to 1993 than the period 1968 to 1982. Rates of growth and mortality were generally highest in serai species and lowest in main canopy taxa. Mortality rates were also highest amongst the smallest trees enumerated (dbh <30 cm). Changes in composition and structure over the survey period are believed to reflect forest recovery after low intensity pit-sawing was curtailed from 1951. Forest recovery also may have caused the reduced growth rates and turnover recorded for the most recent survey period, by restricting the opportunities for light-demanding, faster-growing and relatively productive serai taxa, and to have outweighed any effects of externally-driven processes, such as changes in atmospheric conditions.
New Approach for the Synthesis of IV-VI Thermoelectric Thin Film Materials and Devices on Si: (211) PbSnSeTe/ZnTe/Si Heterostructures

Low-dimensional thin-film thermoelectric materials including superlattices and quantum-dot heterostructures have shown significant potential for improving the thermoelectric properties. Highly structured thin film materials such as these would be useful for integrated microdevices such as power MEMS and thermally triggered MEMS actuators. For reasons including scalability and ease-of-integration, silicon is the choice material for the substrate, however there is a lattice and a thermal-expansion mismatch. In this work, a new approach for the synthesis of integrated PbSnSeTe based thermoelectric thin film materials on silicon is demonstrated by employing an epitaxial buffer layer of II-VI compound telluride materials (e.g., ZnTe, CdTe) to help bridge the lattice and thermal expansion mismatches with silicon. This multilayer can be used for subsequent growth of thick films having low-dimensionality structures. We report the initial results from studying the structural and thermoelectric properties of simple solid-solution alloy PbSnSeTe thin films on ZnTe/Si heterostructures. Data from transmission electron microscopy and in-situ electron diffraction will be presented that shows that despite the large lattice mismatch with silicon and the three different crystal structures, unusually high structural quality PbSnSeTe has been obtained by matching the (211) lattice symmetry and the lattice spacing along the [110] directions of the ZnTe and PbSnSeTe. The structural quality of the PbSnSeTe was studied by measuring the dislocation density through etch-pit counting and x-ray diffraction. Results presented will show that a dislocation density as low as 1.2 × 106/cm2 can be achieved by strategic lattice-matching between the buffer and thermoelectric layers. Electrical resistivity, doping density, and Seebeck coefficient values for solid-solution alloys without low-dimensional structuring will be shown to approach those of high-performance bulk.

Background
In spite of the virtually ubiquitous nature of the initial 10-day placebo run-in period (IPR) in drug trials, there is little empirical data establishing its relevance.
MethodData from 593 subjects were examined retrospectively to determine whether or not the prognosis of subjects minimally improved during the IPR was different to those who were unimproved. The IPR period was single-blind and was followed by a six-week double-blind phase in all studies.
ResultsTwenty-six per cent of the subjects were minimally improved and 74% were unimproved. Approximately 10% of the subjects who were much improved were not followed systematically. Across a range of diagnosis, severity and chronicity subjects minimally improved (versus unimproved) after IPR had a more favourable prognosis whether assigned to drug or placebo.
ConclusionsChange during IPR appears to be a meaningful predictor. Stratification should be considered in future antidepressant studies.

Background
Investigation of the geographical distribution of schizophrenia and its relationship to socio-demographic factors is useful for planning services.
MethodIndividuals with schizophrenia (n=980) were identified by key informants within an inner London borough and point prevalence calculated for broad, Feighner and DSM–III–R schizophrenia. The distribution of cases was tested for significant variation using the Poisson process model. Regression models using the Jarman-8 score and its component variables were tested for their ability to predict the prevalence of schizophrenia.
ResultsA high point prevalence of schizophrenia (5.3 per 1000 resident population) was demonstrated. Case distribution showed a marked and significant variation associated with socio-demographic factors. The prediction of prevalence was more accurate for broad than for narrower definitions of schizophrenia; unemployment rate performed best.
ConclusionsUnemployment rates and Jarman-8 scores may provide crude estimates for resource allocation in planning mental health services, highlighting the need for additional services in deprived inner city areas.
